Boss Toe is developed when the second toe is further forward than the big toe. This is caused by the positioning of the metatarsal bones in the foot. Usually, these form a smooth curve across the foot and the toes are aligned correctly. In some cases, the metatarsal bone behind the big toe is shorter than usual. This forces the trajectory of the curve to alter and break away from the smoothness. This does not cause a problem if the second toe is shorter than the big toe, for the big will still extend the furthest. Many people can have Boss Toe and suffer no discomfort from it.

However, if the second toe is longer than the big toe, Boss Toe is developed due to the way the second toe will extend further than the big. This causes additional pressure on the second toe and can cause discomfort and pain in the ball of the foot.
